September 18, 2014 Siemens Recalls Audible Fire Alarm Base Due to Risk of Injury

This recall involves the SBGA-34 audible base that is affixed to ceiling-mounted smoke detectors in order to sound an alarm when the fire alarm system is activated. The audible base has part number S54370-F13 and date codes 0113 through 2314 in a WWYY format printed on a white label on the back of the unit part affixed to the wall. "MODEL SBGA-34" is printed on a blue label also affixed to the back of the device. The base is off-white and measures about 6 inches in diameter. September 11, 2014 Kidde Recalls Smoke and Combination Smoke/CO Alarms Due to Alarm Failure

This recall involves Kidde residential smoke alarm model i12010S with manufacture dates between December 18, 2013 and May 13, 2014, combination smoke/CO alarm i12010SCO with manufacture dates between December 30, 2013 and May 13, 2014, and combination smoke/CO alarm model KN-COSM-IBA with manufacture date between October 22, 2013 and May 13, 2014. They are hard-wired into a home's electric power. The i12010S and i12010SCO come with sealed 10-year batteries inside. The KN-COSM-IBA model has a compartment on the front for installation of replaceable AA backup batteries. The alarms are white, round and measure about 5 to 6 inches in diameter. Kidde is engraved on the front of the alarm. Kidde, the model number and manufacture dates are printed on a label on the back of the alarm. "Always On" is also engraved on the front of alarms with sealed 10-year batteries. Read more.


August 13, 2014 ESL, Interlogix Hard-Wired Smoke Alarms Recalled Due to Failure to Alert Consumers of a Fire

This recall involves thirty-three models of the 400 and 500 series Edwards (ESL)-branded and Interlogix smoke detectors hard-wired into a security system. Edwards and Interlogix smoke detectors are identical. The 2-wire or 4-wire detectors are round, white and measure 6 inches across by 2 inches high. The smoke detectors were made for professional installation and used primarily in commercial buildings, schools, hotels/motels, apartments, dormitories and homes as part of the fire alarm system. Recalled units have date codes 13084 through 14059 with the first two digits representing the year, for example: 13 for 2013 and 14 for 2014; and the last three digits representing the day of the year, for example: 084 is the 84th day of 2013, which is March 25, 2013. The initials ESL, the date code and one of the following model numbers are printed on a label on the back of the detector's cover and on the product's packaging. 400 series: 429AT, 429C, 429CAD, 429CRT, 429CST, 429CT, 429CTAD, 449AT, 449C, 449CRT, 449CSRH, 449CSRT, 449CST, 449CSTE, 449CT and 449CTE. 500 Series: 511C, 518C, 521B, 521BXT, 521B-10PKDMP, 521B-10PKG, 521BXT-10PKG, 521BXT-DMP-10PKG, 528B, 528CRXT, 541C, 541C-10PKG, 541CXT, 541CXT-10PK and 548C. May 21, 2014 Nest Labs Recalls to Repair Nest Protect Smoke + CO Alarms Due to Failure to Sound Alert

This recall includes all Nest Protect Smoke + CO alarms. The alarms are made of black or white plastic and are about 5.25 inches square with rounded corners and about 1.5 inches deep. The word "nest" is on a large button on the face of the alarm. Consumers can enable the alarms to be controlled by a computer or a smartphone over a wireless network. The alarms have a Nest Wave feature, which allows users to temporarily silence some alerts or cancel a manual test by vigorously waving an arm near the unit that triggered the alarm. Nest Protect Smoke + CO alarms came from the factory with the Nest Wave feature enabled and with the slider button in the "On" position in Nest Protect Settings. Read more.


January 28, 2014 System Sensor Recalls Reflected Beam Smoke Detectors Due To Failure to Alert Consumers in a Fire (Recall Alert)

This recall involves System Sensor's reflected beam smoke detectors with model number BEAM1224S and date codes 2111 through 3053. The YMMW format date codes stand for (2111) 2012-November-1st week of Nov. through (3053) 2013-May-3rd week of May. The detectors are ivory and black and measure 10 inches high by 7½ inches wide. The model number and date code are printed on a label on the back of the detector's cover and on the product's packaging. The reflected beam smoke detectors were used primarily in commercial buildings as part of the fire alarm system. Detectors used with acceptable power supplies, as listed on the company website, do not need to be replaced. Read more.


January 16, 2014 Bosch Security Systems Recalls Fire Control Panels Due to Fire Alarm Failure (Recall Alert)

The recall involves G-series Fire Alarm Control Panels that are professionally installed and have model numbers that end in GV4 and use D192G Notification Appliance Circuit (NAC) modules. The NAC module monitors the circuit connections and signals when alarms are not operational. The Bosch name, logo and model number D9412GV4, D7412GV4 or D7212GV4 are on the board of recalled GV4 fire alarm control panels. The control panels and modules are used in residences and commercial facilities and can be mounted inside a variety of enclosures with a minimum size of 16 inches x 16 inches x 3.5 inches, which may or may not be labeled with the Bosch name or logo. February 15, 2012 Fire Control Panels Recalled by Bosch Security Systems Corp. Due to Alarm Failure Posing a Fire Hazard

The Bosch-branded fire alarm panel is locking red wall box with dimensions 22.7” high by 14.5” wide by 4.3” deep. The status, date and time can be seen through a glass screen on the panel door. The word “BOSCH” is printed on the right corner of the panel and the model number “FPA-1000-UL” is printed on the bottom left below the glass screen. The alarm panels featured software versions 1.10, 1.11 and 1.12, which can be determined by installers. They were designed to be used in small to medium-sized facilities in both public and residential buildings. Read more.


August 9, 2011 Emergency Exit Signs Recalled by Best Lighting Products Due to Failure to Illuminate

The recalled exit signs have emergency lights fixed at either end with "EXIT" in red or green in the center. The fixtures are installed in commercial buildings, such as hotels and office buildings. Affected model numbers can be found on a label affixed inside the fixture housing and on the outside of the packaging, and include: LHQM LED G M6, LHQM LED R M6 and LHQM LED R HO M6. Affected models have the following date codes on the inside fixture housing: 1003S10, 1004S10 or 1005S10. Dates on the packaging carton include any calendar dates in March, April or May 2010. February 8, 2011 Simplex Fire Alarm Control Panels Recalled by Tyco Safety Products Westminster Due to Failure to Alert Monitoring Centers

The recalled fire alarm panel is the Simplex 4100U-NXP panel configured for third party monitoring and running software revision 14.01. The words "Simplex," "4100U" and "Fire Control" appear on the fire alarm's front panel. The software revision number can be identified by pressing the "Menu" key on the panel keypad, selecting "Show Software Revision Level" and pressing "Enter." Read more.
