March 26, 2013 West Music Recalls Egg-Shaker Toy Instruments Due to Choking and Aspiration Hazards

These recalled egg-shaker toy instruments are plastic, egg-shaped instruments sold in five colors: yellow, green, blue, red and purple. The toy instruments are about the size of small eggs and have a "Basic Beat" label printed on the front of the product. The product is marked for children ages 3 years and older but West Music is concerned with the possibility of the egg being used by or near a younger child. The toy instruments have Lot Number 0E0212 located on the bottom of the roundest part of the egg.

March 21, 2013 tc electronic Recalls Bass Guitar Amplifier Due to Electrical Shock Hazard

This recall involves tc electronic 250 W bass guitar amplifiers with model BH250 and serial numbers 1204763 through 12404375. The amplifiers are about 8.6 inches wide, 2.5 inches high and 9 inches deep and have a red front panel. The model and "tc electronic" are printed on the front of the amplifier. The serial number is located on a white sticker on the back of the amplifier.

August 16, 2012 Heated Mats for Pets Recalled by World Class Due to Fire and Electrical Shock Hazards to Consumers; Sold Exclusively at Amazon.com

This recall involves "GSI electronic 50W indoor pet heating comfort pad mats" with model number GKC2G002. The mats are polyester, were sold in various shapes and sizes and have a white electrical cord. They were sold in a variety of colors, including beige and pink. Read more.


May 31, 2012 English Riding Supply Recalls Wire Mouth Bits for Horses Due to Risk of Injury

This recall involves eight models of Happy Mouth wire mouth bits used with a bridle in horseback riding. The bits involved in this recall have steel braided wire running through the plastic mouthpiece and most with a metal ring on each end. Model numbers included in the recall are 462172SS, 462177SS, 462181SS, 462184SS, 464123SS, 466898SS, 466904SS and 467248SS. Model numbers can be found on a label attached to the product. “Happy Mouth” is engraved on the plastic mouthpiece. Read more.


June 2, 2011 Musical Shaker Instrument Recalled by Woodstock Percussion Due to Laceration and Choking Hazard

The shakers are plastic musical instruments, 5.5 inches long, with rounded, egg-shaped tops containing steel pellets and open circular handles. They come in blue and green and are sold separately. The marking “B4” is printed on the rounded top of each shaker. Read more.

April 21, 2011 Aquarium Heaters Recalled by United Pet Group Due to Fire and Laceration Hazards

The recall involves the following Marineland Stealth and Stealth Pro aquarium heaters. The heaters are black plastic tubes and have a temperature adjustment knob at the top. The model name "Stealth" (in white letters) or "Stealth Pro" (in red letters) is printed on the side of the heater. The model number and the wattage rating are printed below the model name. This recall involves the nylon carrying case sold with Night Train guitar amplifiers. The black carrying case has metal clasps and a nylon arm strap. “VOX” is printed on the front pocket of the carrying case. The carrying cases were sold with amplifiers with model number NT15H. The recalled products are Lennox Superior brand VFGL Log Sets and VF4000, VF5000 and VF6000 fireplaces. Log sets are comprised of ceramic-fiber logs with a dual gas burner system and are designed to be placed in a wood-burning fireplace or a ventless firebox enclosure. Fireplaces are comprised of log sets, a ventless firebox enclosure and accessories. Each product has a metal rating plate attached to the grate of the log sets or to the frame of the fireplaces containing the unit’s model number, serial number and other information.
