This recall involves previously recalled Matrix Fitness Ascent trainers® and Elliptical trainers installed from November 2011 through December 2012. All Ascent trainers and ellipticals have a swing-arm handle design, with pedals that rotate in an elliptical path. Units also have stationary handlebars. The Ascent Trainers® are similar to the elliptical machines, but include an incline function. Users operate the machines in a standing position. The ellipticals are black and silver-colored. The Ascent trainers are also black and silver-colored with an orange accent color on the machine's body and foot pedals. "Matrix" is printed on the machines body and handlebars. When assembled, the machines are about 70" high x 29" wide and 68" long. The machines involved in this recall have a decal at the center/bottom of the units with model numbers beginning with A-3x, A-5x/7x, or E-3x/5x/7x. This recall includes units with serial numbers beginning with EP304, EP306, or EP308, and with date codes of 1208 (YYMM) or prior. Read more.

This recall involves the CloudCharge Qi Wireless Charging Pad. The charging pads were a promotional giveaway to employees and customers of various companies.The product name and model number T4706 are printed on the bottom of the product and select company promotional logos and artwork printed on the top. The recalled chargers are circular and have a white plastic bottom with the top having various promotional artwork. They measure about 4" in diameter and .5" tall. A white USB power cable is included along with a wireless receiver adapter. Each charging pad and accessories are packed in a white plastic box with a clear lid, measuring approximately 5" x 5" x 1.25". This recall involves the Orange Whip golf swing trainer models OWT and OWM. The recalled trainers have a black grip and flexible shaft with a weighted orange ball attached to it. The batch number is printed on the inside of the counterweight steel ball (white or gold color) attached to the grip end of the Orange Whip. The recalled trainers have a batch number in the range from 772 to 792. In order to read the batch number, the counterweight steel ball must be unscrewed by hand. This recall involves Okie Dokie Denim Patches Shortie shorts. These jean shorts have a rainbow and a slice of watermelon patch on the front of each pocket with tan stitching along the seams. The shorts were sold in toddler sizes 2T through 5T. The item number 324-8191 and date code 12/17 are printed on a white tag on the inside back seam of the shorts. Read more.

This recall involves Bronze Hammered bistro chairs. The chairs measure 37 inches tall; and have a round seat and a heart-shaped backrest. SKU number 400167187741 can be found on a tag attached to the chair or on a sticker on the underside of the seat. Read more.

This recall involves Crate and Barrel's Josephine floor mirrors, sold individually or as a set of three mirrors that can be hung vertically or horizontally. The mirror consists of a larger rectangular mirror and a smaller square mirror on wooden backing that measures about 16 inches wide, 3 inches deep and 90.5 inches long. This recall involves two styles of children's sleepwear garments. The first garment is a 100% polyester fleece pajama pant with the brand name "Sweet N Sassy" on the waist label and hang tag. The style number is located on the back of the brand label at the center waist. The second garment is a 95% polyester and 5% spandex onesie with long-sleeves, a hood and front zipper with the brand name "Delia's Girl" at the neck label and hang tag. The style number is located on the back of the brand label at the neck. The garments were sold in children's sizes 4-6x and 7-16 in a variety of colors and prints. This recall involves Ortovox avalanche transceivers, model numbers 1137000006, 1137000001 and 1137000002. They were sold in black/blue (model number 1137000006), black/black (model number 1137000001) and neon green (model number 1137000002). The transceiver is used as a beacon to locate an individual in the event of avalanche burial. ORTOVOX and 3+ are printed on the top of the device. In the open/receive position, the top half of the transceiver displays a blue screen with green border that displays an image and distance reading of the buried individual. The transceivers measure about 5 inches long by 3 inches wide by 1 inch thick in the closed/transmit position. All transceivers manufactured from 2010 through 2018 are included in the recall. The manufacture date is printed inside the battery door on the back of the transceiver with a roman numeral representing the quarter of the year and a two digit number referring to the year. For example, a manufacture date of IV/17 represents the fourth quarter. The year of manufacture is five years earlier. In this case, the transceiver was manufactured in the fourth quarter of 2012. This recall involves model year 2018 Camso DTS 129 and Yeti Snow MX kits that are used to convert off road motorcycles or dirt bikes to track and ski vehicles. The DTS 129 kits have a serial number starting with 9025ICEXXXX located on the conversion system frame. The Yeti Snow MX kits have a serial number starting with YE3XXXX located under the system.
